EQ-5D-3L | Self-complete version for use in Qualtrics

Qualtrics is one of the world’s biggest survey platforms that is used by commercial and non-commercial researchers to handle everything from simple questionnaires to detailed research projects. You will find more information on Qualtrics Core XM Survey Software here.

Qualtrics versions of the EQ-5D-3L are available in a few languages. If a language version is not available, please contact the EuroQol Office.

Please note that:

• Due to Qualtrics licensing restrictions, we can only provide these modules to non-commercial customers. Commercial customers will have to create their own EQ-5D representation in Qualtrics.
• EQ-5D in Qualtrics requires a Qualtrics CoreXM Survey Platform, with “Allow Javascript permission” enabled. Without Javascript the survey will not work. Free Qualtrics accounts do not allow Javascript but Professional license and up will. Also bigger non-profit accounts (e.g. universities and hospitals) often do allow it.
