Can I still use and/or request the Proxy 2?

Effective 1 August 2025, the EuroQol Office has discontinued offering the Proxy2 as a separate mode of administration (MOA).

Since the wording of Proxy 1 is largely similar to Proxy 2—differing mainly in the instructions—it will be permissible to adapt the Proxy 1 by modifying its instructions to reflect the perspective of the respondent rather than the proxy’s own opinion. This modification is allowed upon signing a Permission to Modify. This means users may continue to collect data using the Proxy 2 approach by adapting the Proxy 1 accordingly.

To request this alternative option, please complete the EuroQol Registration form, select the Proxy1 version, answer the question Are you planning to modify EQ-5D? with yes, and add in the text field: request to modify Proxy 1 to Proxy 2.

Ongoing and New Studies

Proxy 2 versions currently in use as part of ongoing studies may continue to be used. However, for any new studies starting after 1 August 2025, the adapted Proxy 1 must be used instead, as the EuroQol Office will no longer provide, distribute, or support the Proxy 2 versions beyond this date.
