2. Complete the form with missing information.
3. Proceed to the Summary tab of the registration form.
4. Copy all information on the summary screen into an email and send it to userinformationservice@euroqol-oud.falkor.alcor.cloud. Please state in the email  that you were not able to submit your registration using the online form.
