Can I still use and/or request the Proxy 2?

Effective 1 August 2025, the EuroQol Office has discontinued offering the Proxy2 as a separate mode of administration (MOA).

However, users may continue to collect data using the Proxy 2 positioning by modifying the Proxy 1 version accordingly.

If you would like the proxy to record how they (the proxy) think the person (e.g. patient or care recipient) would report their own health if the person themselves was able to communicate it (hereafter referred to as a ‘Proxy 2’), it is recommended to:

  • Confirm that the objective is truly seeking the proxy’s understanding of what they believe the person themselves would respond if they were able to (rather than simply the proxy’s perspective of the other person’s health – Proxy1 version), and then
  • Request permission to modify the Proxy 1 to accommodate the desired ‘Proxy 2’ positioning via the EuroQol Registration Form.

Since the wording of Proxy 1 is largely similar to Proxy 2, it will be permissible to adapt the Proxy 1 to reflect the perspective of the respondent rather than the proxy’s own opinion. When modifying the Proxy 1 version to achieve Proxy 2 positioning, there are 10 text rows of the Proxy 1 versions that are recommended to be changed. When requesting the modification, a legal document (Permission to Modify) with detailed instructions on how and which lines need to be modified will be provided.

To request this alternative option, please complete the EuroQol Registration form, select the Proxy1 version, answer the question Are you planning to modify EQ-5D? with yes, and add in the text field: request to modify Proxy 1 to Proxy 2.

Ongoing and New Studies

The already delivered Proxy 2 versions currently in use as part of ongoing registered studies may continue to be used. However, for any new registrations of studies after 1 August 2025, the adapted Proxy 1 must be used instead, as the EuroQol Office will no longer provide, distribute, or support the Proxy 2 versions beyond this date.
