EuroQol Experimental Instrument Registration Form

The EuroQol Office introduced the EuroQol Experimental Instrument Registration Form on the EuroQol website on the 1st of June to streamline all incoming requests for the use of Experimental versions. The registration form applies to requests to use the EQ-HWB, EQ-HWB-S, EQ-TIPS and EQ-5D-Y-5L which are not publicly available yet and are only shared with researchers who are willing to serve as a collaborator on their development. Collaboration is possible on fundamental research about the instruments. The research performed by collaborators is not expected to generate findings with an immediate application.

Applicants can submit a request for collaboration by completing this registration form. Requests will be reviewed by the EQ-HWB/EQ-TIPS/EQ-5D-Y-5L developer group and may be accepted or rejected. In case the applicant is accepted as a collaborator, the applicant may collaborate in the EQ-HWB/EQ-TIPS/EQ-5D-Y-5L developer group on the further development of the EQ-HWB/EQ-TIPS/EQ-5D-Y-5L instruments. The collaborator will receive a Collaborator Status and will be required to sign legal documents.
