Digital on Qualtrics

Modular versions for Qualtrics are only provided to non-commercial users.

Please note that: EQ-5D in Qualtrics requires a Qualtrics CoreXM Survey Platform, with “Allow JavaScript permission” enabled. Without JavaScript the survey will not work. Free Qualtrics accounts do not allow JavaScript, but Professional license and up will. Also, bigger non-profit accounts (e.g., universities and hospitals) often do allow it.

If you are combining this survey with another Qualtrics survey or creating a new Qualtrics survey that includes questionnaires outside of EQ-5D, be aware that the JavaScript used for the VAS question might not work as expected.

The instructions provided in the User Portal provide details on how to use EQ-5D in Qualtrics. 

EuroQol will not provide technical support when deviating from these instructions, including any changes in JavaScript, questionnaires, or layout functionality.
